Tobacco Cessation Toolkit Workgroup
Health Care Provider and Community Partner Survey:
ESD 123 is gathering information on the types of resources and information healthcare providers and community organizations utilize and need the most when offering commercial tobacco cessation services to their patients and audiences.
We ask that you please take this survey to help us learn more about the tools that are most helpful for you to support those who want to quit smoking or vaping (e-cigarettes) commercial tobacco and other nicotine products. This survey is estimated to take about 15 minutes, thank you for your participation!
Deadline to complete the survey is February 28, 2023.

We want to hear from the community! When it comes to quitting smoking or vaping commercial tobacco and other nicotine products, what types of messages, tools, and materials are helpful to families and their loved ones? Help us receive feedback that will help us create better resources and support for the community. All answers are anonymous, we will not collect personal identifiable information or contact participants directly without prior approval.
If you would like paper copies of the survey, English/Spanish, please email Briseida Chavez - [email protected], a prepaid stamped envelope will be included for your convenience. You can also drop off surveys at ESD 123 office - 3924 W Court St Pasco, WA 99301 between 8-4 PM. Please address to Briseida Chavez.

Prevention Loteria nights: We offer an opportunity to engage influential adults and youth in prevention education by doing a prevention loteria night at your family nights! Please email Briseida Chavez at [email protected] if you would like to collaborate on hosting a Prevention Loteria night with your families!
